Effectiveness of Gabapentin for Sciatica

     Gabapentin is approved in the United States for the treatment of epilepsy and postherpetic neuralgia. He has then prescribed gabapentin 300 mg once daily; the pain improved significantly, even after the first dose. The drug is gradually titrated up to 900 mg three times daily with good results. The patient then underwent a laminectomy and discectomy on the advice of his neurosurgeon, who assured him that the results would provide immediate pain relief. After surgery, the patient continues to feel pain; However, the pain completely disappeared after a few weeks of taking gabapentin 600 mg three times a day. 

    The second patient was a 68-year-old Caucasian woman with renal failure who experienced severe burning pain and tingling pain with sharp onset in the back of her right leg; Sciatica was diagnosed. The patient has contraindications to NSAID therapy and intolerance to hydrocodone. Initial therapy with propoxyphene and paracetamol initiated by the patient alone was ineffective. The patient's pain improved rapidly and after a follow-up visit approximately 5 weeks later, he had good pain control with gabapentin. 

    Gabapentin is commonly prescribed to treat peripheral neuropathic pain syndrome. However, as far as we know, these two cases are the first in which sciatica with gabapentin has been successfully described. Since gabapentin has the potential to prevent central sensitization, prescribing this therapy should be considered early in the course of sciatica. Additional studies with randomized placebo-controlled trials are needed to confirm the benefit of gabapentin in the treatment of sciatica.
